Elder Care Services

We create personalized care plans for your loved one’s hands-on care. Your Case Manager checks in frequently to assess progress and changes. This includes an initial fall risk evaluation and ongoing home safety and care assessment.

Our staff are trained and licensed healthcare providers who are hand-selected to ensure they meet our high standards for personalized and patient-centered care. Each in-home health provider is screened and trained to ensure they are aligned with Accurate Care’s high quality of standards. Our client care plans are personalized to address current care needs, but also to take a proactive approach to ongoing client needs. Your loved one’s care team reports any changes in health, which are immediately passed along to you. This includes educating and training you on how to best support your loved one in their home.

Skilled In-Home Care Providers

Registered Nurses

Your assigned RN will perform an in-home evaluation and assessment to create a personalized care plan for medication management, wound care, managing health conditions, and more. This includes ongoing monitoring and updates.

Certified Nursing Assistant

Your assigned CNAs provide hands-on assistance for activities of daily living including bathing, dressing, hygiene, and nutrition. They help maintain independence in the comfort of your loved one’s home.

Physical Therapists

Physical therapists play a vital role in maintaining independence and quality of life. They help your loved one maintain and regain the mobility required for maximum independence.

Speech Therapists

As we age our vocal cords can weaken, our ability to swallow can decline, and health conditions can negatively impact speech. Our speech therapists work with your loved one to improve their communication.

Occupational Therapists

Our occupational therapists work with your loved one, teaching them exercises and how to use support aids to complete daily activities of living with as much independence as possible.

Licensed Clinical Social Workers

Your assigned social worker will advocate on behalf of you and your loved one, suggesting elderly resources throughout the entire Chicagoland area. They will also provide emotional support for you and your family.
